There are three (3) types of forms on which to record your information. Each form has a brief description under the field label to help you determine the content of that field.
Agency Update Form |
Use this form to enter the main Agency Information Please be sure to indicate the "Control Person for Agency Information" as this will be our future contact for obtaining information updates. |
Program Update Form |
Use this form to add information about each individual program the agency offers. Please fill in the Agency Name in case the forms become separated during the data entry process. |
Service Update Form |
Use this form to add information for each individual service the agency offers. Please fill in both the Agency and Program Names for the same reason. |
Use the following definitions to help determine which forms your agency needs to use. There should be only one Agency Update Form completed. You can use multiple Program and Service Update Forms as they fit your needs.
The Regional Information and Referral provider who is responsible for ensuring that your data is correct and current in the new Community Information and Referral Internet Database (CIRID). In this case, that is the St. Albert Community Information and Volunteer Centre.
The official/legal name for your agency/club/association/organization. All references to you for Internet Database purposes shall be as an Agency.
The name by which you call an individual activity or related group of activities. For example:
A sporting association offers different levels of activity depending on the age of the participants. The program names could then be atom, peewee, midget, etc. or Ages 10-12 Program, or Butterfly Group, (whatever name you would call this breakdown of activity level).
The name by which you call individual activities provided under one program. For example:
A sporting organization offers activities at a recreational and a competitive level, separated into age groups within those categories. The program names could be recreation and competition with service names under the program such as children, youth, adults, etc.
An indicator used to determine what data can be made public, used by the Regional Information and Referral providers or used only internally. When the indicator is set by the Agency Name, this mode will apply to all information for the agency. Several fields also allow you to indicate the publish mode on them individually while still allowing general information to appear publicly.
